Dentures

Few people want to have their teeth extracted just to look better. But when restoring the teeth is impossible because there is simply not enough tooth structure left, dentures are an alternative.

It is important from a cosmetic standpoint, however, that you need not allow too much time to lag between tooth loss and tooth replacement. Tooth loss causes the face to collapse and the mouth to sink in. At the same time, the nose appears to drop down towards the chin. Deep lines begin to from in the creases of this collapses skin, adding years to the face.
Full dentures can make you look younger because there is almost no limit to what the dentist can create with them.

Immediate Dentures :

Immediate dentures are measured for size with the natural teeth intact, and they are placed the same day as your remaining teeth are extracted. With this type of denture, you skip the try in stages that accompany the fitting of other models. Because gums and bone tend to shrink after extraction, these dentures may become loose.

Fitting conventional dentures typically requires three to six office visits after all of the teeth have been removed and the tissue has healed. It is up to you during these visit to pass judgment on the fit and look of the dentures. Your dentist will advise you on color, tooth type and lip position. If the color is too white, the dentures will look artificial.

Immediate Removable Complete Denture :
After extraction of teeth, shrinkage of bone takes place for the first 4-5 months and hence a temporary denture is given to the patient. This is called as immediate denture. This denture should be used for 4 to 6 months and should be replaced.

Cast Partial Dentures

Advantages :
» Patient can enjoy his social life i.e. if he waits for 6 months after total extraction of teeth, he has to move around without teeth.
» Patient can chew better i.e. as the wound healing has not taken place, he cannot chew hard food but this denture helps him to eat better.
» From medical point of view, this denture helps to stabilize his lower jaw i.e. if the patient stays without teeth for six months, his lower jaw drifts forward.
Removable Complete Denture :
When gums and bones heal properly, removable complete denture is prepared. There are 5 sittings.
1. First Sitting - Primary Impression :
This sitting takes only 10 minutes. Impression of gums and jaws is made with the help of trays. The trays used to make the impression are of standard prefabricated sizes.
2. Second Sitting - Final Impressions :
This is a very important step. It decides the fit of a denture. The tray used to make this impression is specially fabricated for every individual. This consists of two stages :
1. Border Molding
2. Zinc-eugenol paste impression.
3. Third Sitting - Jaw Relation :
It decides position, size, shape of teeth and hence, is a significant step in denture making. With the help of wax a relation between upper and lower jaw is noted. This relation is sent to the laboratory.
4. Fourth Sitting - Trial :
With the help of the relation of jaws taken previously, a denture in wax is made where all the teeth are placed in wax. This trial denture is placed in the patients mouth. This enables the patient to note the size, shape, color, occlusion and overall appearance of his teeth. Any changes regarding his teeth can be fixed at this stage, hence is a decisive stage.
5. Fifth Sitting - Denture Delivery :
A polished, finished denture is delivered to the patient. Instructions of how to use a denture are given to the patient.
6. Follow - Ups :
A new denture may hurt, like a shoe-bite and hence may require little adjustments.

A new type of denture gaining popularity is the Implant retained denture that can be used to replace some or all teeth. This type of denture comprises metallic implants that are embedded into the bone and give support for artificial teeth.

Denture Hygiene :

Dentists advise all the patients to always rinse the dentures thoroughly under running water as follows:

  1. A. Soak dentures daily with an immersion cleaner following the manufacturer's direction.
  2. B. Clean your dentures a second time each day using a denture cleaning paste and a denture brush. Do not use toothpaste.
  3. C. Scrub your dentures weekly with a thick mix of cleaner containing bleach. Rinse thoroughly when completed.
  4. D. If heavy calculus (tartar) deposits form on your denture - soak overnight in white vinegar, brush area carefully in the morning and rinse thoroughly. This procedure may be used on partial dentures.

When handling your dentures do so over a sink filled with water. This will cushion them should they slip and fall. When your dentures are being cleaned, rinse your mouth thoroughly with water. Brush your upper and lower jaw ridges and tongue with a soft bristle tooth brush. Finger massaging of the ridges is also helpful.

Sore Spots :

Appearance of some sore spots in your new dentures is not unusual. These will be taken care of by your dentist during the first few adjustment appointments. As time goes by changes in your jaws or your occlusion (bite) may also cause sore spots which can be relieved.

Speech :

Practice, practice, practice!!! Initially some you may have incoherent speech. Be patient and your speech will improve. Practice reading aloud until your speech is clean and natural.

Dislodging :

Sudden bursts of air such as coughing, sneezing and laughing; very wide opening such as yawning; or extreme tongue movements will dislodge even the best dentures. Avoid these movements or place your hand over your mouth.

Adhesives :

Use denture pastes and powders only as directed by your dentist.

General Advice:

Be sure to leave your dentures out overnight or for at least 2 hours each day. Dentures should be stored in a covered container with the cleaning solution when not in the mouth.

Generally the upper denture will be better than the lower. One must learn how to use and retain the lower denture.

In the beginning, do alone, as much talking, reading aloud and eating as possible. This will allow you to avoid embarrassing moments as you learn to use your new dentures.

Avoid very hot food, your temperature sensitivity will be changed by the dentures covering your jaws.

In time, due to continued resorption of the jaw, your dentures may require relining.

Have your dentures, mouth, and adjacent structures checked by your dentist at least once a year. The needs and nature of each patient vary, therefore, check with your dentist about anything not covered here.

Dentures can be regarded as the best dentistry treatment option for the people who can not choose dental implants or the orthodontic treatment for some reasons like age or health conditions. Dentures can also fulfil the need of a cosmetic dental treatment giving the patient a better appearance, better look and the confidence back again.
